When you register, you will receive a complete fundraising kit, which provides sponsor sheets, ideas for raising funds, letters to request pledges, etc. – everything you need to make your fundraising a success! If you need additional sponsor sheets, you can make your own! Just be sure to 1) include the sponsor’s name and contribution; and 2) make a photocopy for your records before you turn your sheet in.

REGISTRATION FEES:

$10 per Family Team member $___________ ($15 after 10/14/00)

Each family team member must complete a registration form. All registration forms and fees must be received together to qualify for Family Team discounts.

Photocopies of the printed form or printouts of this form are acceptable.
